C4 Application Form
Form C4 (headed 'Application for an order for disclosure of a child's whereabouts') has four simple sections.
1 About you (the applicant)
Fill in your name, address, date of birth and relationship (e.g. father) to each child.
2 The child(ren)
Fill in the name, date of birth and sex of each child and any identifying details (enclose a recent dated photograph if possible). Give the name and last known address of the person believed to have control of the child.
3 The order you are seeking
Fill in the names of any person or agency to whom the order should apply - the Department of Social Security is one obvious choice - and specify when and how you want the information disclosed (e.g. 'by post as soon as possible').
4 The grounds for the application
State why you believe that the court does not have adequate information and why you believe that the person or agency specified would have any relevant information. Be brief - any complicated details can be submitted in an affidavit.
Sign and date the form, make a photocopy for your files, and take the original to the court office with your fee. The form is available from the Court Service website.
