C3 Application Form

 

Form C3 (headed 'Application for an order authorising search for, taking charge of, and delivery of, a child') has three simple sections.

1 About you (the applicant)
Fill in your name, address, date of birth and relationship (e.g. father) to each child.

2 The child(ren)
Fill in the name, date of birth and sex of each child and any identifying details (enclose a recent dated photograph if possible). Give the name, address and telephone number of the person believed to have control of the child.

3 The grounds for the application
State whether the application is ex parte and if so why. Give details of the order that is being disobeyed and the best available information as to the whereabouts of the child.

Sign and date the form, make a photocopy for your files, and take the original to the court office with your fee. The form is available from the Court Service website.
